Social Services - Amend an agreement (DSS3012-B-21) with Westhab, Inc., for which Westhab was to operate the Vernon Plaza Family Center as a Tier II Shelter for Families, as defined in 18 NYCRR Part 900, as well as operate an emergency overnight shelter within the Vernon Plaza Center, for adults with children, on an as-needed basis, as such need is determined by the County, for an aggregate amount not-to-exceed $5,817,554, for the period from 2/1/21 - 1/1/24, and amend the agreement by, 1.) extending its term by one (1) year, to a new termination date of 1/31/25; 2.) increasing its amount by $2,017,264, to a new total not-to-exceed amount of $7,834,818, to pay for the Services during the Second Option Term, and 3.) reflecting the fact that the County will have the sole option to further extend the term of the agreement for one (1) additional one-year period thereafter, for the not-to-exceed amount of $2,057,609, on the same terms and conditions set forth in the agreement, for the Third...
