Corporation in order to permit additional New York State Housing Finance  
Agency Subsidy Funds in the amount of $11,855,954 into Schlobohm  
Apartments, Schlobohm Homes (aka Palisades Towers) located at 1-8  
Schroeder Street and 72 Palisade Avenue (Section 2, Block 2030 Lots  
49, 50, 300, 325; Section 2 Block 2026 Lots 200, 225, 250 and Section 1

funds being provided by NYS HFA, as well as increase the amount of a  
prior lien of which the County funding will be subordinate to in accordance

1/1/2026 - 12/31/2026, with the exception of the NSIP Grant Agreement,  
which will commence retroactively on 10/1/2025 - 9/30/2026.

supportive housing for homeless or formerly homeless adults with one or  
more disabilities including mental illness and/or chemical dependency  
disorders, in the amount of $15,533,360, for the term from 6/1/2026 -  
5/31/2027; and (ii) Enter into 6 sub-recipient agreements with the listed  
agencies to provide permanent supportive housing for homeless or  
formerly homeless adults with one or more disabilities including mental  
illness and/or chemical dependency disorders for the period from  
6/1/2026 - 5/31/2027 in a total aggregate amount no to exceed  
$15,533,360.

Open Door Care Network to provide expansion of primary medical care  
and preventive health services and outreach services for its most  
vulnerable uninsured and underinsured patients from 6/1/2026 -  
12/31/2026 in an amount not to exceed $100,000.

Debra L. Acocella and Nicholas A. Acocella, and AltaVista Lead  
Improvement, Inc., the Contractor, to perform lead remediation services

New Rochelle, NY in accordance with the New York State Leading in  
Lead Prevention Pilot Program for the period upon execution of the  
agreement through 3/31/2027 in an amount not to exceed $40,000; and  
(ii) enter into a Local Program Administrator Agreement with the  
Participant, to provide an amount not to exceed $40,000 to assist the with  
the direct costs of lead remediation, as well as any eligible related costs,  
in exchange for Declaration of Interest secured as a recorded lien on the

County to enter into an agreement with St. John’s Riverside Hospital to  
provide the Maternal & Child Health Partnership Pilot Initiative Program to  
address and improve Black maternal and child health outcomes and  
reduce racial health inequities in the County, in order to (i) remove a  
previously authorized subcontractor; (ii) authorize new subcontractors;  
and (iii) due to the removal of said subcontractor and authorization of  
new ones, re-distribute the funds allocated.
