Non-Emergency Moving Services for Homeless Customers of the

term thereof for the period from 6/1/24 - 5/31/25; and 2) increasing the  
not to exceed amount thereunder by an additional $47,615.29, payable at  
the new rate of $317.43 per move, based upon the change in the C.P.I.,  
for services to be rendered during the extended term.

Service Society of Yonkers “(FSSY”) to provide Family Caregiver Support  
Services for County grandparents/relative caregivers, 55 and older of  
children under 18 years old, under Title III-E of the Older Americans Act,  
as amended and its implementing regulations, in order to extend the term  
of the agreement for the program period commencing retroactively on  
1/1/24 - 12/31/24, Federal funding for the Second Extension Term will be  
in an amount not-to-exceed $45,100, payable based upon the  
agreed-upon unit-costs for actual services provided and data-entered in  
the New York State Office for the Aging’s PeerPlace System, and that  
FSSY is required to contribute $15,034 in Contractor matching funds for  
the Second Extension Term.

from 7/1/24 - 6/30/25; and 2) enter into an agreement with JRB 360, LLC,  
for which JRB 360, LLC will serve as an Anti-Violence Coordinator to  
oversee the implementation of the GIVE program’s goals and initiatives  
and facilitate the monthly GIVE meetings and the quarterly GIVE  
Executive Board meetings, as well as other services, for a term  
commencing on 7/1/24 - 6/30/25, payable monthly at the rate of $7,500  
per month, not-to-exceed the aggregate sum of $90,000.
