Social Services - Amend agreement with Westhab, Inc. pursuant to which Westhab was to provide services to operate a facility at 101 North Broadway, Yonkers, New York in order to, (1) extend the term by six (6) months, to a new termination date of August 8/31/2026; (2) increase the not-to-exceed amount by $469,605.50, from $5,391,666 to $5,861,271.50, in order to pay for the Services during the extended term; and (3) modify the budget to specify the allocation for the extended term ("The Seventh Amendment", DSS312-G-19).
