Health - Agreement with Sun River Health, Inc. to conduct outreach to re-engage its uninsured and underinsured patients as well as engage new clients without a current medical home, to provide primary medical / dental care and preventive health services with targeted outreach to women ages 18-44 for linkage to preventive care, for the period from 6/1/2026 - 12/31/2026 in an amount not to exceed $100,000.
