Community Mental Health - Amend an agreement with Westhab, Inc., for the provision of certain community mental health services, for the period from 1/1/22 - 12/31/26, for an amount not-to-exceed $3,295,335, in order to, a.) modify its scope of work in order to add the provision of additional community mental health services, for the period from 1/1/24 - 12/31/26; b.) increase its amount by $278,181, to a new total amount not-to-exceed $3,573,516, in order to pay for the Additional Services; c.) modify its budget in order to allocate the additional funds that will be used to pay for the Additional Services; and d.) modify its scope of services in order to specify that the services under code 0690 (Outreach (Non-Licensed Program) for the period from 8/1/23 - 12/31/26, with no change in cost associated with the modification.
